PAYMENT SCHEDULE

• For THEATRE PROJECTS, full tuition is due May 15,

2008. Deposits and tuition payments are not refundable

after May 15, 2008. Casting is not announced

until payment is made in full. To hold a place in a

theatre production, you can make a deposit of $200.

The deposit is applied to your final tuition payment.

• Please return this form with a check or money

order made payable to MadCAP. Please NOTE:

THIS YEAR THERE WILL BE A 5% UPCHARGE

FOR USING A CREDIT CARD

• For cancellations before May 15, 10% of the payment

or deposit is retained as a processing fee.

• Extended day enrollment forms are sent with

confirmation. Fees for extended day are $3 per hour

for early drop-off or late pick-up with a minimum

one hour payment for each scheduled time. Extended

day arrangements must be made in advance.
